GLP-1 long term side effects

Two different questions get asked under this heading, and they have different answers. One is what happens to the well-known gastrointestinal effects over time. The other is what is known about extended use. Only the first has a clear structural answer.

These products carry a boxed warning regarding thyroid C-cell tumours observed in rodent studies, and are contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2. On the first question: gastrointestinal effects are generally reported as concentrated around dose increases and easing at a stable dose. That is why the titration schedule is stepped, and why people who have been at a maintenance dose for months usually describe a very different experience from their first weeks.

Common questions

What does the labelling say about long-term use?
These are chronic-use medications for chronic conditions, and the prescribing information addresses ongoing use. Specific long-term data is a matter for the published literature and your prescriber, not for a summary page.
What about muscle loss over time?
Loss of lean mass accompanies large weight loss regardless of method. Protein intake and resistance training are the levers usually cited. This is a consequence of the weight loss rather than a direct drug effect.
What about hair loss?
It is on the label with numbers: 3.3% on Wegovy 2.4 mg against 1% on placebo, and 7.1% of women against 0.5% of men on Zepbound. Both labels attribute it to weight reduction rather than to the drug directly. Covered in full on its own page.
What happens when someone stops?
The medication works while being taken. What follows depends on why it was started and is a question for the prescriber who started it.
What is the boxed warning?
Thyroid C-cell tumours observed in rodent studies, with contraindication in personal or family history of medullary thyroid carcinoma or MEN 2. This is screened for before starting rather than being a long-term development.
