Without coverage, the cheapest route depends on which structure you use, not which pharmacy you walk into. There are four distinct structures, and they are quoted in units that cannot be compared until converted.
Where the price comes from
The routes people actually pay through, and what determines the number in each.
Through insurance (copay)
If your plan covers the drug for your indication, your cost is the plan copay. Coverage for weight-loss indications is far less common than for type 2 diabetes.
Telehealth platform (brand drug)
Online clinics bundle a prescriber visit with brand-name medication. Membership fee is usually separate from drug cost.
Compounded semaglutide / tirzepatide
Compounded versions are prepared by pharmacies rather than the original manufacturer. Availability and legality depend on FDA shortage status, which has changed several times — verify current status before relying on this route.
Retail pharmacy cash price
List price without insurance, sometimes reduced by discount cards.
That structure also answers the "where do I go" version of this question. There is no single cheapest supplier to name, because the routes are not competing on the same product: a manufacturer self-pay programme, a telehealth membership that bundles the prescriber visit, and a retail cash price are three different purchases. Picking the route comes first; picking within it barely matters by comparison.
The comparison that actually matters is cost per milligram at the dose you expect to maintain on — not the introductory monthly figure. Programmes routinely quote the starting step, where consumption is lowest. At maintenance you may be using several times as much drug for the same headline price.

- Both major manufacturers run direct self-pay options for people without coverage. Tiers and eligibility change frequently, so check the official programme page rather than a third-party summary.
- Are compounded versions still the cheap option?
- That has changed substantially. Large-scale compounding of these molecules has narrowed sharply since 2026 — see the compounded semaglutide page on this site for the current regulatory position before assuming this route is available.
- Do discount cards work on GLP-1s?
- Retail discount cards apply to cash prices and their effect varies by pharmacy and by drug. Worth checking, but rarely the difference between affordable and not.
- What is the single biggest lever?
- Coverage. If your plan covers the drug for your indication, your cost becomes a copay and bears no relationship to any list price. Establishing that is worth more than any amount of shopping around.
- What is the average cost without insurance?
- No average worth quoting exists, and any figure published here would be wrong within weeks. The spread between routes is wider than the spread within any one of them, so an average across all of them describes nobody. Enter the quotes you were actually given above instead.
- Can I get a prescription online without insurance?
- Telehealth services prescribe without requiring coverage — that is the model. What you are buying is a programme fee that bundles the prescriber visit with the medication, which is why it cannot be compared to a per-vial pharmacy price until both are converted to cost per month.
- Does the dose I end up on change the answer?
- More than anything else on this page. A quote that looks cheapest at the starting step can be the most expensive at maintenance, because a fixed quantity of drug lasts a different length of time at each step. Always run the comparison at the dose you expect to stay on.

- Why do quoted GLP-1 prices vary so much?
- Because the quotes are not measuring the same thing. A membership fee, a per-vial price, and a per-pen price all describe different quantities, and the dose you are on determines how long that quantity lasts. Converting everything to cost per milligram is the only way to compare them directly.
- Does insurance usually cover this?
- Coverage is far more common for type 2 diabetes indications than for weight management. Whether your specific plan covers your specific indication is something only your plan documents can answer.
